Our office is open from 9 a.m.-4:30 p.m. Monday-Friday.
Closed 12-1pm Daily.
Your RELY Health Care On-Call Nurse will be available to you 24 hours a day,
through our answering service to help with any of your health care needs.
No charge for nurse assessment.
We offer you fully trained personnel that have been interviewed and insured.
RELY provides PRI Screening which is the service need in pre-admission to nursing homes.
